Fill out your paperwork before your visit! Below are four documents that all new patients must complete prior to their visit. Download and print the forms, complete them at your leisure and bring them to your visit! By filling these out before your visit, you will reduce your wait time in the office by 15-20 minutes!!
Medical Records Request
If you are transferring your care from another dermatologist or if you would like Dr. Wilson to review your medical history, please download and print this "Medical Record Release" form. This form gives your previous physicians permission to send a copy of your records to our office.
