2. Call us at 502-316-9425 to schedule a virtual visit or email us at info@elkhorndermatology.com; please provide us with your chosen platform and associated contact information.

3. A visit will be scheduled.  You will receive an email with information about your virtual visit and a telehealth consent form.  This consent must be verbally acknowledged at the time of virtual check-in for your visit.
